Why an island clinic works differently
Koh Phi Phi has no hospital, no roads and no ambulance. The nearest emergency department is
roughly two hours away across open water, the scheduled ferries run in daylight only, and after
dark a crossing means chartering a boat.
Between about sunset and morning, then, a clinic that is actually open is the only medical care
on this island. That is not a marketing position, it is the arithmetic of where we are, and it is
why the doors do not close.
What that means in practice
A cut at one in the morning gets repaired at one in the morning, while it can still be closed the
simple way. Somebody whose friend cannot be roused gets assessed at three rather than at breakfast.
Treatment that runs on a seventy-two hour clock starts tonight instead of losing ten hours to
nothing. And a fever climbing at midnight gets a
blood test rather than a long wait and a guess.
Who we look after
Divers and dive professionals, backpackers, families on the beaches, and the substantial number
of people who work this island through the season and rarely see a doctor otherwise.
Most of our patients have no medical records with them, do not speak Thai, and want to understand
what is wrong and what it costs before agreeing to anything. That is the normal case here rather
than the awkward one.
What we can do on the island
- Walk-in consultations at any hour
- Urgent and emergency care, including oxygen, IV medication and adrenaline
- Same-visit blood tests — dengue, malaria, full blood counts
- Burn treatment, including the fire show injuries this island is known for
- Stitches, wound care and minor surgery
- IV drips for dehydration, stomach illness and heavy nights
- Rabies vaccination after a Monkey Beach bite
- Dive medicals and assessment after a dive that did not feel right
- Doctor hotel visits on foot across the village
What we cannot do, said plainly
We are a clinic, not a hospital. There is no X-ray here, no CT scanner, no operating theatre, no
blood bank, no inpatient beds and no recompression chamber. Anything needing imaging, surgery,
transfusion, admission or recompression goes to Krabi or Phuket.
Our job in those cases is the part that actually changes the outcome: stabilise you, call ahead so
the receiving hospital is expecting you, and arrange the boat and the ambulance so the two connect
rather than you waiting on a pier for either. That is
evacuation and hospital transfer, and it is a
real service rather than a disclaimer.

Costs, and the paperwork
You are told what something costs before it is done. Payment is at the end of the visit, cash or
card, and you leave with an itemised receipt and an English medical report — the two documents
almost every travel insurer asks for.
If you dive, check your policy before you need it. A great many travel policies exclude scuba
diving outright or cap it at a depth well below what you are certified for, and a good number also
exclude fire-related activities and injuries sustained after drinking. See
